“Passports in the Gardens”: The City of Miami Gardens is a Passport Processing facility for the U.S. Department of State

MIAMI GARDENS – The Office of the City Clerk for the City of Miami Gardens will be on hand, Saturday, April 18, 2009, from 8:00 a.m. – 12:00 p.m. to accept and process Passport applications, at the City of Miami Gardens City Hall, City Council Chambers, 1515 NW 167th Street, Building 5, Suite 200.

If you are not able to make that date, the Office of the Clerk is also open Monday through Friday – 9:00 am – 4:00 p.m. to accept Passport Applications. Preferably by appointments, however walk-ins will be accepted.

Miami Gardens City Hall is located at 1515 NW 167 Street- Suite 200, Miami Gardens, Fl. 33169.

All those wishing to apply for a U.S. Passport (both children and adults) MUST:

— Apply in person (all infants, children, adults and the elderly must appear in person) *

— Be a U.S. citizen (born in the United States or be a naturalized U.S. citizen)

— Bring their original Naturalization Certificate or Birth Certificate (NO photocopies!)

— Provide two (2) color passport photos (for each person applying for a passport)

— Show valid government-issued photo identification (such as Florida Driver’s license)

— Each person over age 14 must pay $75 by personal check or money order made out to The “U.S. Department of State” to apply for a passport; for children under 16 years old, the cost for each passport application is $60. Cash and credit cards are not accepted.

— Each adult and child must have completed (but MUST NOT SIGN) an “Application

For a U.S. Passport” (Form DS-11) available at the North Dade Regional Library, Miami Gardens City Hall, or on-line at the State Department website: www.travel.state.gov.

* All children under the age of 16, in addition to appearing in person, must be accompanied by BOTH their parents (mother and father). If one parent is not available to personally accompany the child, that parent must complete — and have notarized — a “Statement of Consent or Special Circumstances; Issuance of a Passport to a Minor Under Age 14” (Form DS-3053) and bring that form with you. You must also bring with you a “long form” birth certificate showing the name of the child, as well as the name of BOTH parents.

The only situations where only one parent can accompany the child is if they have proof of sole custody (such as a court order or divorce decree granting the one parent sole custody, or if only that one parent’s name appears on the birth certificate, or if there is a death certificate proving that one parent is deceased). All documents must be originals (NO photocopies).

Those who apply for a U.S. passport in the Gardens” should receive their U.S. passport, via priority mail, within 6-8 weeks from the day they apply. The Executive processing fee for each application is $25.00, which much be paid by separate check made out to the City of Miami Gardens. In addition, there are no long lines and no waiting.
